Print the Team Member Evaluation Form from the course Website, under "Projects". Fill out the form per the instructions below. Submit in class on the due date above.

This form is required

Each member of the team is expected to contribute equally to each part of the project. Each team must turn in a paper copy of this form (print from Web, under Projects) to report the relative effort/contribution of each person on your project team over the whole project. This form is not optional. Be professional and give a
careful rating. The ratings on these forms will be used as weightings, as explained at the beginning of the semester, to convert team project grades into individual student project grades.

Rating of Contribution/Effort

Please give your rating as a kind of weighting factor, based on a nominal rating of 100%. I.e., a 100% rating means that a team member gave 100% of what was reasonably expected from that team member. So, if your team members all contributed essentially the same amount, then each member is assigned 100. However, if you believe someone did more than their share or someone did not carry their fair share of the load, you are professionally obligated to indicate that here. This is your way, and ours, to make sure a fair evaluation is made for effort rendered. Thus, if a team member gave only 80% of the effort/contribution of what was expected, you
would assign that team member 80 as a rating. The numbers do not have to add up to an average of 100% for each team member. In rare cases you may rate a team member more than 100%. For any member you do not rate at least 90%, you must explain your rating in the comments.

Confidentiality

Your ratings and comments will remain anonymous outside of being read by the instructor, and possibly in some cases by the GTA. However, the general thrust of the overall set of evaluations for a team might have to be discussed with a team member. Use the comments area of the form to convey anything we should know
regarding team work and group dynamics. For example, did one of your partners dominate the decision making and directing of the project to the extent that it diminished the learning experience for you? Team Member Names (include yourself) Rating (Effort/Contribution) (Nominally = 100% each)

How you evaluation will be used

The team is given a grade for each part of the project. Each individual team member's grade for each project assignment is a weighting of the team grade, where the weighting is based on an evaluation of individual contributions, collected via this form from each team member at the end of the semester.

Deliverable

Hand in a single sheet of paper (separate from the project report notebook), your individual team member evaluation form.

Grading

No credit toward the project grade will be given for this simple assignment, but late submission (or no submission) of this assignment will result in points deducted from your overall project score.
